Camp Medical Forms

Camper medical information, including vaccination records, must be updated every year, as per NYS law. Baltimore Woods does not keep medical records from year to year. The medical forms are completed as part of the registration process. Vaccination records DO need to be submitted annually via official documentation from the physician’s office. 

Please submit your camper’s vaccination records a minimum of two weeks in advance of the camp session which they are attending. Campers without complete medical information will not be admitted to camp. 

If you do not have a copy of your child’s vaccination records, the easiest thing to do is to call their doctor’s office and have them fax the records directly to Baltimore Woods at (315) 673-3671.

How to Submit Vaccination Records:

Option 1 – Direct from Physician

If you do not have a copy of your child’s vaccination records, the easiest thing to do is to call their doctor’s office and have them fax the records directly to Baltimore Woods at (315) 673-3671.

Option 2 – Upload Record into your Active Network Account

If you have your child’s vaccination records handy, you can scan them into a computer, or even take a photograph of them, and upload the files directly into your Active Network Account. Here’s how:

  • Step 1. Sign into your Active Account. You can access your camp account by clicking here. You will need to use the same email address and password that you used when you registered for camp.

  • Step 2. Look for the blue box labeled “Upload Form”, and click it (#2 arrow in picture). Follow the prompts for uploading the vaccination records (i.e. “supplemental forms”)

The Parent Account Access button takes you to your online account with Active Network.  The online account provides accessibility for registrants to update personal information, review registration details, print statements and make payments on existing balances.
